Posterior Reversible Encephalopathy Syndrome Presenting with Thunderclap Headache

Abstract
Hypertensive encephalopathy usually presents as posterior reversible encephalopathy syndrome (PRES), which is characterized by symmetrical vasogenic edema in the parieto-occipital areas where sympathetic innervation is relatively deficient compared with other areas. PRES is a neurological disorder of acute onset characterized by varied neurological symptoms, which may include headache, impaired visual acuity or visual field deficits, disorders of consciousness, confusion, seizures, and focal neurological deficits. We herein report a 57-year-old man presented with thunderclap headache. There were no abnormalities in cerebrospinal fluid examination and brain computerized tomography. Magnetic resonance imaging revealed abnormalities in the white matter in the parieto-occipital regions. PRES can present as a thunderclap headache.
